Personal Health & Medical Form Class 3 (48KB in size Adobe .PDF file) For use by ALL Scouts and Scouters coming to Spanish Peaks Scout Ranch. Use this form or an equivalent as a health form to participate at SPSR. (See page 16 in the SPSR Saddle Bag.) The State of Colorado requires the Class 3 form to be used.

Merit Badge Sign Up Forms, Parental Permission, and Informed Consent Form (Adobe Acrobat PDF File) It is required by Colorado law for parents or guardians of children attending summer camp sessions at Spanish Peaks Scout Ranch to give permission for a child to take part in activities and merit badges at the scout ranch during the summer session. This document is a parent permission form to be filled out by parents and guardians of scouts attending camp. It is returned to the scoutmaster to evaluate and set up on rosters with the merit badge schedules and then sent to Spanish Peaks Scout Ranch BEFORE THE TROOP ARRIVES IN CAMP. This form along with the scout’s personal health form is to be kept on file at the camp for state recording requirements.
